When you are diagnosed with avascular necrosis (AVN), the treatment conversation often jumps straight to an operation: drill the bone now, or replace the hip later. Those are real options, and sometimes the right ones. But they are not the only ones. At AVN Arogya, AVN is our flagship speciality, and in the majority of the patients we treat for early-stage AVN, progression is arrested without surgery; many of our patients who were advised replacement have avoided or deferred it.
What decides the right treatment for avascular necrosis
Three things matter most:
- The grade. Whether the ball of the hip is still round (Grades 1 and 2), has begun to collapse (Grade 3), or has flattened with arthritis (Grade 4).
- The cause, and whether it is still active — steroids, alcohol, a clotting tendency or another condition.
- You — your age, your general health, how much pain you have and what your daily life demands.
If you are not sure of your grade, our guide to reading your hip MRI or X-ray report will help.
Conventional treatment for AVN: the options you will hear about
This is how AVN is usually managed in orthopaedic practice.
- Painkillers and anti-inflammatory medicines ease the pain but do not change the course of the disease.
- Other medicines — such as bisphosphonates or blood-thinning and cholesterol-lowering drugs in selected patients — are sometimes used, with mixed evidence for slowing collapse.
- Reducing weight on the hip with crutches or a walker, for a period.
- Core decompression, an operation that drills one or more channels into the ball of the hip to relieve pressure inside the bone, sometimes with a bone graft or bone-marrow cells. It is used mainly before the bone has collapsed. We explain it, and the questions to ask, in Core decompression for AVN.
- Bone grafting or osteotomy — operations to support or reposition the bone, offered to selected younger patients.
- Total hip replacement, when the joint has collapsed with arthritis.
Conventional care, in other words, mostly protects the joint mechanically or replaces it. Strengthening the bone's own capacity to heal is where Ayurveda adds a further dimension.
Avascular necrosis treatment without surgery: what Ayurveda adds
The body has a powerful capacity to heal, and Ayurveda strengthens it with medicines and therapies that work differently from modern drugs. Bone is living tissue; when the blood flow and the conditions for healing improve, it can stabilise.
At AVN Arogya the approach is to find the root causes through a thorough history and diagnostic tests, then address them one by one. The treatment generally works to:
- reduce inflammation, including the swelling in the bone marrow that causes the deep groin pain
- improve blood flow to the affected bone
- strengthen the bone by supporting bone metabolism
- remove whatever is getting in the way of the body's natural healing
How Ayurvedic treatment for AVN works at AVN Arogya
It starts with a consultation. The physician looks at your whole history and reports, and decides whether our treatment suits you and how long it is likely to take. You can send your MRI in advance for a preliminary evaluation.
The first course is an inpatient programme, usually 21 days and occasionally extended to 28. Even mild cases need this inpatient start, followed by outpatient care.
There is no fixed protocol. Every programme is designed personally by your physician for your grade, your inflammatory state, your body and how you respond — an approach refined across four generations of AVN practice. It draws on:
- our own Ayurvedic medicines, made in-house and tested to international standards for heavy metals
- classical therapies chosen for you, such as Panchakarma procedures including Vasti, and Kerala therapies such as Navarakizhi, Elakizhi and Podi kizhi
- physiotherapy and yoga, to strengthen the muscles around the hip without overloading it
- a therapeutic diet designed by our in-house dietician
Then structured follow-up. You go home with a home medicine programme, monthly teleconsultations with your physician, a repeat MRI at six months, and an optional seven-day booster stay each year.
Results of Ayurvedic treatment for avascular necrosis
Our patients usually feel the pain ease first during their stay, then walking and daily activities improve over the following months.
- Grades 1 and 2: in the majority of the patients we treat, progression is halted, with almost complete pain relief and recovery of function to near normal levels.
- Grade 3: in the majority of the patients we treat, function and pain improve significantly and hip replacement is avoided, with no further progression in stage. Read more in AVN stage 3: can it be treated without surgery?
- Grade 4: many of our patients gain meaningful pain relief and function and are able to postpone surgery for years with continuous care.
Progress is measured with a Harris Hip Score, a pain score and a quality-of-life questionnaire before and after treatment, with an expert radiological review a year later. You can watch our patients describe their recovery in their own words.
When surgery is the right treatment for AVN
We say this plainly. Grade 4 AVN with severe pain and disability — being unable to walk or manage daily life — despite a full course of treatment followed faithfully, usually needs a hip replacement. Healthy patients over 70 are also good candidates for replacement. When surgery is the right path we recommend it clearly, refer you to orthopaedic surgeons, and support your recovery afterwards. Choosing surgery then is not giving up; it is the right treatment.
Frequently asked questions
Can avascular necrosis be treated without surgery?
Often, yes, especially when it is found early. In the majority of the patients we treat for early-stage AVN, progression is arrested without an operation. Whether it suits you is decided at consultation, after your physician has seen your MRI.
Which is better for AVN: Ayurveda or core decompression?
They work differently. Core decompression relieves pressure inside the bone mechanically; our treatment works on inflammation, blood flow, the causes and the body's own healing. Many patients choose an Ayurvedic evaluation first, especially in the early and intermediate stages; if your hip needs surgery, we will tell you.
Can I continue my allopathic medicines during treatment?
Yes. Our physicians ask about every medicine you take and advise you which ones to continue. Never stop or change a prescribed medicine, especially steroids, on your own.
Will my insurance cover AVN treatment?
Inpatient treatment at our Vilachery hospital in Madurai can be cashless through more than 35 insurance companies and TPAs, with interest-free EMI available.
Seek urgent care if your hip pain suddenly becomes severe, if you suddenly cannot put weight on the leg, or if the hip is hot and swollen with a fever.
For an honest second opinion and a chance to avoid unnecessary surgery, consult an AVN Senior Physician and bring your MRI. Read about Ayurvedic treatment for avascular necrosis at AVN Arogya, or book a consultation — you can send your MRI and reports in advance.
